Personal Information

1.	Enter your First Name.

2.	(Optional Step) Enter your Middle Name.

3.	Enter your Last Name.

4.	Enter your Email address.

5.	Re-enter your email address to Verify Email.

6.	Enter a User ID. This ID must be between 2 and 50 characters ( periods, dashes, and @ are allowed).

7.	Enter a Password. The password must be at least eight characters long and contain three of the four types:

•	Lower case alpha character (a-z)

•	Upper case alpha character (A-Z)

•	Numeric character (0-9)

•	Special character (e.g. ! @ $ % ^ & *)

8.	Re-enter your password to Verify Password.

Security Identification

1.	Select a Security Identification Question for Questions 1, 2, and 3. You must select a different question for each.

2.	Enter your answers to security Questions 1, 2, and 3.

3.	Click Next. This takes you to Step 3. -OR-

Click Back to go back tot he previous page. -OR-

Click Cancel to cancel your account request.
