The General information includes the following contact information:
| • | USDOT - The number assigned by MCMIS to a census record. This is sometimes referred to as the USDOT number or USDOT#. Each company should have only one active census number. The census number has no internal coded structure. Numbers are issued sequentially as companies/shippers/registrants are added to the system. |
| • | EIN - The Employer Identification Number. |
| • | SSN - The Social Security Number |
| • | DUNS - Provided by Dun & Bradstreet, a DUNS number is used by the government to identify contractors and their locations. |
| • | Entity Type - This identifies the entity as a carrier, hazardous materials shipper, both a carrier and a shipper, or registrant. |
| • | Legal Name - The legal name of the company. |
| • | DBA Name - Any name, other than the legal name, that identifies the company. This may be a trade name under which the company does business. |
| • | E-mail - The company's e-mail address. |
| • | Company Address - The address where the company's principal place of business is physically located. |
| • | Company Phone - The company's primary telephone number. |
| • | Contact Cell Phone - The cell phone number for the company's primary contact. |
| • | Company Fax - The company's fax phone number. |
| • | Mailing Address - The company's address where it receives mail. |
| • | Status - The company's status. |
